Kisco or Pleasantville, NY FLSA: Non-exempt STATUS & HOURS: Full-Time- 35 Hours per week Date Submitted/Revised: October 2023 JOB SUMMARY: The program’s Social Worker/Case Planner is responsible for the comprehensive service coordination from initial assessment to obtaining appropriate and adequate services for each resident. This includes planning the resident’s services, linking provider to persons in need of service, monitoring the services received in light of a resident’s service plan, and coordinating all phases of services, such as vocational, educational, residential and day programming. The social worker also arranges for appropriate transition services for a youth nearing discharge from the program. GENERAL RESPONSIBILITIES: Write treatment plans, court reports, progress notes and individual crisis management plans.
