*** THIS POSITION MAY BE ELIGIBLE FOR UP TO A $10,000 SIGN-ON BONUS (restrictions may apply) **** At Nascentia Health the Registered Dietitian will be responsible for the delivery and evaluation of medical nutrition therapy services to agency patients. Requirements: - Provide in-home consultation for policy and program development and operation and evaluation of nutrition services. - Serve as a consultant to the agency in the application of nutrition knowledge. - Provide direct service to patients and families through home visits. Assess nutrition status and food practices; assist in plan for the provision of food and fluids appropriate for the individual's physical and environmental conditions. Provide nutrition education and counseling to meet nutritional needs. - Provide appropriate educational materials to staff and participate in in-service training of staff. - Demonstrate knowledge of and ensure compliance with agency policies and procedures. Maintain patient confidentiality. - Demonstrate knowledge of, and ensure compliance with, federal, state, and local regulations governing home care. - Complete nutrition care documentation in a timely manner.
