Hospice Nursing Assistant CNA | On Call / PRN | Duluth MN area. The Nursing Assistant, CNA is responsible for providing personal care and designated health-related services as outlined in the clients care plan. This position is responsible for following the direction of the nursing staff by following the individualized care plan for the clients they care for. We are seeking a compassionate CNA/HHA who is looking for a change of pace in their CNA career and is interested in patient-centered, one-on-one care. This role provides the opportunity to support hospice patients in various settings, including private homes and senior living or skilled nursing facilities. The ideal candidate values meaningful connections, dignity at end of life, and teamwork. Reliable transportation is required, as this role involves travel between patient locations.
