Description JOB SUMMARY: Within the context of a patient/client and family-centred framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, the Occupational Therapist / Physiotherapist Case Manager provides case management service to a select population of clients and families experiencing brittle support systems, family conflict, social, cognitive and/or physical functioning decline, and/or mood and behaviour issues. Duties include conducting assessments that considers psychological factors and the surrounding social environment on physical and mental wellness and their ability to function in home and community care, providing individual and family/caregiver support, making recommendations about care, maintaining client records, and attending interdisciplinary clinical/case conferences. Identifies applicable community resources for clients, contributes to the effectiveness and development of community resources and planning with clients for care and support in accordance with their health needs.
