Job description

Responsible for ensuring provision of quality nutritional assessments and education to Center’s patients through individual and group educational activities. Assists Director of Population Health in the development of nutritional programs for Center. DUTIES AND RESPONSIBILITIES: 1. Plan and provide individual and group nutrition counseling and education to Center patients. 2. Assess the dietary and health needs of patients, guide patients to make lifestyle changes utilizing motivational interviewing, provide Medical Nutrition Therapy for appropriate patients, promote nutrition through public speaking and community outreach programs, and keep abreast of the latest research in nutritional and food sciences. 3. Educate patient and family members on nutrition, diet, food selection and food preparation. 4. Consult with other Center staff (physicians, nurses, health educator, and counselors) concerning the patient' food habits and dietary needs and serve on inter-disciplinary team for chronic disease education and programs. 5.
