Position: District Nurse Casual positions available: Classification: Depending on qualifications & experience. About the position: Applications are invited from Registered Nurses who would like to take up the opportunity to join our dynamic Community Health team to work in District Nursing team in Yarrawonga. The primary role of the District nurse is to provide a person centred approach focusing on maximising functional capacity, home based care, improved self-management strategies and coordination of service delivery to clients in their own homes. Successful applicants will be supported with an internal orientation and training and will be supported throughout by the Clinical Coordinator of Community Nursing, District Nursing colleagues and the Operational Director of Community Services. About you: You must be committed to providing evidenced based practice, demonstrate an understanding of and commitment to nursing in the community setting and quality improvement. Applicants should have the appropriate skills including the ability to plan care, assess clients, and liaise with other care providers.
