Schedule: Mondays: 9:30am – 6:00 pm. Tuesdays through Fridays: 8am – 4:30pm. Position Details: Lifecare provides outpatient adult HIV treatment and prevention services in a multidisciplinary clinical setting located at Methodist Hospital. The Case Management Specialist is part of the Social Work team that supports the patients of Lifecare and is an integral part of the health care team. Location: Methodist Hospital: Department: Lifecare Position Requirements: With 15 hospitals, including seven with Magnet designation and eight with Pathways to Excellence designation, our team members are leading the way through excellence. - High School Diploma/GED required. Associate's Degree preferred. - 1-3 years of experience required. - Requires the ability to comprehend third party contractual arrangements. - Requires basic computer skills including the ability to enter progress notes and other data in multiple medical record systems.
