The Medical Case Manager is responsible for providing case management services to persons with HIV/AIDS as directed by policies and procedures of the HOPE Program and associated funding agencies. This position is for the Bethlehem Clinic. Bilingual capability-Spanish language preferred. JOB DUTIES AND RESPONSIBILITIES: - Provide case management services to persons living with HIV/AIDS.
