Description Community Services Provider: Part-time hour 6-29/per week: Wage: 17.50/per hour: The Community Services Provider is responsible for providing direct care through implementation of Habilitation Plans. Specific duties and responsibilities include, but are not limited to: - Provide supervision and all direct care required to maintain safety and comfort of the person receiving services - Transport or accompany people receiving services on trips to community outings, appointments, or recreation activities for the purpose of community integration and exploration - Development and planning of inclusive community activities - Establish and maintain positive relationships with area businesses - Assist people receiving services with completing daily or routine domestic or personal care tasks - Foster and promote skills leading to independent living - Assist with the development of Habilitation Plans and following these plans for training and assistance - Provide training and assist people receiving services to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required - Provide supervision to people supported during program...
