STATEMENT OF JOB: The LPN/RN provides quality nursing care and services to residents in a respectful and dignified manner, ensuring the delivery of care and services. The LPN/RN coordinates physical, social, and emotional aspects of assisted living care and services in accordance with the physician's orders and the resident's choices. Responsibilities include but are not limited to: - Execution of the medication management plan including periodic review of the documentation, observation of the effectiveness of the plan, communication to the physician when changes are needed or outcomes are not appropriate - Continuous assessment of resident needs with updates to appropriate individuals as it relates to care, services and changes in condition - Implement measures to ensure resident wellness and safety and the appropriate documentation of those measures - Work in a team fashion to meet resident needs - Respond to emergencies timely and in a calm and competent manner - Report incidents and changes in resident condition to Healthcare Coordinator and with the appropriate documentation - Be an active part of all activities including community events - Be present and assist with the deliv...
