POSITION SUMMARY: The Emergency Department Admission Avoidance Program seeks to identify, strategize and implement discharge planning as soon as a patient arrives at St Joseph’s Healthcare Hamilton. Admission Avoidance was adopted in 2016 out of great need to better serve the community demand for Emergency Care The Admission Avoidance team allocates in-house, community and provincially funded resources to get patients home sooner and straight from the Emergency Department In 2017 the Admission Avoidance Team received the Sisters of St Joseph of Hamilton MISSION LEGACY AWARD for its exceptional commitment to healthcare and upholding the values of our founders.
