Description Klarus Home Health & Hospice/Jet Health Inc is looking for PRN Licensed Vocational Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the Spring Branch/Canyon Lake, TX region and surrounding areas. The primary function of the LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes. Requirements: Key Responsibilities: - Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse. - Coordinate care with the interdisciplinary group, patient/family, and referring agency. - Assist in the development and implementation of patient care plans. - Monitor patient health and report any changes to the supervising RN. - Administer medications and treatments as prescribed. - Educate patients and their families on health care needs, conditions, and options. - Maintain accurate and timely documentation of patient care and progress.
