Job Description: The Advanced Practice Pharmacist (APP) in the Pharmacotherapy Clinic at John Muir Health plays a vital role in the comprehensive management of patients with a variety of complex and chronic disease states. Practicing under a Collaborative Practice Agreement (CPA) in accordance with California State Board of Pharmacy regulations, the APP is authorized to prescribe, initiate, adjust, and discontinue medications to optimize therapeutic outcomes across a range of specialties. Core responsibilities include assessing medication regimens for safety, efficacy, and adherence; streamlining therapies when appropriate; mitigating drug interactions; and educating patients and caregivers to support long-term treatment success. Pharmacists will perform clinical assessments, order and interpret laboratory tests, and monitor patient responses to therapy to ensure safe, timely, and cost-effective care. The APP collaborates closely with physicians and the care team to ensure continuity of care and achievement of clinical goals.
