An exciting opportunity exists for the right individuals to work as Child & Youth Foster Care Case Manager in Kelowna . The successful candidates will be joining a progressive non-profit organization with growing opportunities. Work collaboratively with the department Director and Program Manager to maintain the program in a fun and flexible environment. Coordinate and manage a caseload of assigned responsibilities. Act as a resource to individuals served, their respective family members or representatives and the care providers. Perform a variety of professional activities in support of the Program including but not limited to overseeing contracted community support homes, connecting with contracted care providers in the community to oversee service provision, coordinating OT, PT and other health care-related services to support the individual in service, collaborating with stakeholders, reporting to Funders regarding critical planning and incidents, assisting new participants with transitioning into the program, overseeing individualized client-centered service plans and ensuring all standards are upheld.
