Job description

Job Description Summary The Mobility LPN reports to the Divisional Director of Clinical Excellence. Under the supervision of a Registered Nurse, the Mobility LPN performs multi-skilled activities to support patient mobility, including, but not limited to, ambulation, transfers to/from chair, assisting patients in practicing exercises or functional activities, and total lifts to chair using lift equipment to achieve desired outcomes. The LPN will be responsible for updating the highest level of mobility performed each shift for assigned patients, as well as updating the mobility documentation, such as but not limited to, AM-PAC scores.
