Job description
The associate/assistant care manager assists the care manager to coordinate the care of Life with Dignity’s clients and their families. Work is supervised by a certified care manager. The associate/assistant care manager helps to implement and monitor the care plan as directed by the primary care manager. The associate/assistant care manager may help arrange for all necessary services and monitor the client in conjunction with the certified care manager. Education A minimum 5 years in a geriatric or mental health service-related field. Requirements - Geriatric Experience: The associate/assistant care manager is aware of the major issues of aging (loss, grief, physical changes, depression, mental status changes) as well as normal changes in health as one ages. Knowledge and experience of interfacing with community resources is required. Experience working in a long-term care setting or hospital is preferred. - Organizational Skills: The associate/assistant care manager has demonstrated in prior experiences the ability to work independently and manage priorities to meet the standards of practice of an agency or a caseload.
