Well-established practice with a supportive team and new clinic space currently under development Summary: Join CentraCare as a Family Medicine Advanced Practice Provider (NP or PA) at CentraCare – Litchfield Clinic, a well-established practice supported by a collaborative care team and a new clinic space currently under development. In this role, you will provide comprehensive, patient-centered care to an established panel of patients across the lifespan, including acute and chronic disease management, preventive services, routine checkups, screenings, immunizations, and personalized lifestyle counseling. This is an excellent opportunity to practice independently while benefiting from the resources, specialty access, and integrated support of one of Minnesota’s leading rural health systems.
