Job Title : Population Health Coordinator Department: Population Health: Location: Riedman Campus 100 Kings Highway South Hours Per Week: 40: Schedule: Monday - Friday 8 am - 4:30 pm SUMMARY: Key member of the practice team who has an understanding of primary care practices and assists in the coordination of care required for optimal coordinated population health. RESPONSIBILITIES: - Documents all patient interactions in a concise manner that is compliant with documentation requirements - Conducts in-bound and out-bound calls including, but not limited to: patient scheduling, screenings, reminder calls, panel management - Assists to schedule and coordinate patient transportation, follow-up physician appointments - Utilizes various reports to track patient referrals and scheduling no-shows for patients in collaboration with practice staff - Provides clerical and / or administrative support to clinical staff and managers for special studies, projects and reports - Provides excellent customer service by serving as a resource to all internal and external customers - Maintains knowledge of all health plan benefits and payor requirements, - Call patients as designated by PCP or clinical...
