Job Summary: Administers personal care and assistance with housekeeping activities of daily living for patients in the home setting. Reports pertinent observations of patient's physical and psychosocial status to Registered Nurse (RN), Therapist or Clinical Supervisor. Job Requirements: Licenses/Certifications: - High School Education/GED or equivalent: Preferred - Bachelor’s Degree or equivalent combination of education/related experience: Preferred - Experience with home health or community background: Preferred - Home Health Aide (HHA) license: Required in CA - Certified Nursing Assistant (CNA) license: Preferred in CA - Basic Life Support (BLS OR HS-BLS OR RQIBLS) Health Care Provider certification: Required - Valid Driver’s License (DL) and must be at least 21 years of age or older...
