Specialty Pharmacist Full Time Ohio AndHealth is on a mission to radically improve access and outcomes for the most challenging chronic health conditions, with the goal of making world-class specialty care accessible and affordable to all. We partner with health systems, community health centers and independent practices to remove barriers to care to ensure all people have access to the care they deserve. We are looking for a Specialty Pharmacist to play an important role at AndHealth. This is a full-time position located at a Community Health Center in Ohio. What you’ll do in the role: - Deliver direct care, therapeutic recommendations, and counseling for patients with chronic conditions via the Patient Management Program, protocols, or collaborative practice agreements. - Review patient profiles for interactions, allergies, or duplications, and recommend evidence-based medication or dosage adjustments. - Partner with Patient Engagement Specialists to secure patient access by managing payor formularies, prior authorizations, and appeals. - Maintain strict documentation for URAC and ACHC accreditation and actively participate in Quality Management programs and improvement projects....
