As our Care Worker (known internally as reablement worker), you’ll support older people aged 55+ with varying care and support needs to live independently in their own home with peace of mind. You’ll make a big difference each day as you help our customers achieve their hopes and aspirations. What you’ll do: Support our customers in their own apartment who have a range of support needs including frailty and dementia. Help to regain or maintain independence and live well in their own home by carrying out every day daily living activities by themselves. Carrying out risk assessments, support planning, goal setting and regular reviews.
