What You’ll Do: As a Home Health Physical Therapist, you’ll manage a caseload of patients from admission through discharge, providing comprehensive evaluations, developing individualized treatment plans, and delivering therapeutic interventions. You’ll collaborate with a supportive interdisciplinary team to achieve the best possible outcomes for each patient.
