Social Worker - Discharge to Assess- Acute Hospital OPAL Teams (Queen Elizabeth Hospital and Good Hope Hospital) Permanent: Golden Hello and retention payment - £1,000 + £5,000 per annum; see below for further details and conditions. Consultation grade - subject to formal evaluation under the Pay Equity Review Working 36.5 hours per week: Working Bank Holiday and weekends on a rota system An exciting opportunity has arisen to join the Acute Hospital Teams within the admission avoidance OPAL services at Queen Elizabeth and Good Hope Hospitals. The successful candidate will work across busy multi-disciplinary teams, supporting high-quality social work practice and, where required, contribute to other areas within the Discharge to Assess (D2A) service. A key element of this role is to strengthen collaborative working, promoting effective multi-disciplinary team engagement within a fast-paced environment.
