For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. STATEMENT OF THE JOB: The Licensed Practical Nurse works as an integral member of the team providing services at agency locations across a range of service lines. The Licensed Practical Nurse is responsible for providing wellness services and health monitoring to meet the behavioral health and care issues affecting those served by the agency’s Integrated Health and Wellness programs. The primary goal of health monitoring is to identify clients at risk due to poor physical health and medical conditions, improve their health outcomes through early identification and care, and prevent lowering their quality of life or early death. Health monitoring goals include: providing health assessments and health monitoring to a wide range of agency clients, proving monitoring services to high risk clients weekly, with the aim of reducing medical hospitalizations.
