The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services. FLSA Status: Non-Exempt Job Role Summary: The Care Coordinator also provides skills training and development to clients as required by medical necessity. In collaboration with the treatment team, the Care Coordinator conducts necessary client assessments as well as formulates, develops and revises treatment plans.The Care Coordinator – Opioid Treatment Program adheres to State opioid treatment program regulatory requirements in the delivery of opioid treatment services.
