Job DetailsJob Location: Community Support Services - Akron, OH 44311Position Type: Full Time (40 Hours)Salary Range: $19.00 - $25.00 HourlyThe Opportunity: We are seeking to hire a Case Manager to join one of our regional treatment teams. This individual enhances the provision of clinical services to clients by offering practical help, building community support, teaching recovery-based skills, and monitoring services. What You’ll Do: Serve on a team built up of fellow Case Managers, Recovery Specialists, and medical staff to navigate the needs and care of your clients, attending team meetings on a regular basis. Monitor and coordinate the delivery of necessary evaluations and assessments in order to identify each person’s needs. Assist individuals to identify and minimize the effects of their psychiatric symptoms, enabling individuals to self-manage their illness. Coordinate and participate in the development of an Interdisciplinary Service Plan (ISP) that shall include services and assistance necessary to achieve specific objectives.
