Description SUMMARY: AxessPointe Community Health Center, Inc. (API) is a Federally Qualified Health Center serving the underserved in Northeast Ohio. API is utilizing interdisciplinary services as an integral part of the medical home model. Our API Pharmacy is a 340b Pharmacy SUPERVISOR : Director of Pharmacy: DUTIES AND RESPONSIBILITIES: Pharmacy Establish and Provide Clinical Pharmacy Services Additional Tasks as Assigned by the Director of Pharmacy. - Accurately and timely, fill all prescriptions. - Review prescriptions for Drug-Drug Interactions, Food-Drug Interactions, Drug Allergies, Drug Misuse, etc. - Maintain patient profiles and pharmacy records. - Counsel patients on their medications. - Supervise pharmacy technicians, pharmacy students, and pharmacy residents. - Participate in 3rd party billing and inventory management operations. - Oversee patient assistance program. - Follow current and new Federal and State Laws, including 340b integrity regulations. - Act as the pharmacist-in-charge. - Identify patients for clinical referral....
