The primary focus of this position is to improve health outcomes, housing stability, and income attainment through coordinated supportive services. The Case Manager assists clients in obtaining and maintaining stable housing by conducting assessments, developing individualized service plans, coordinating rental assistance, and linking clients to medical, behavioral health, employment, and community resources. This role includes home visits, crisis intervention, documentation in HMIS and Avatar systems, and ensuring compliance with federal HOPWA program guidelines.
