Department: 39602 Aurora Family Service - Supportive Counseling Services Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: This full-time position will be in-person at that Center for Senior Health and Longevity at the Aurora Sinai Outpatient Center, Mon-Friday during typical clinic hours. No on-call or weekend expectations. Pay Range: $30.70 - $46.05 Major Responsibilities: - Performs culturally responsive psychosocial screening and assessment to identify social, psychological, behavioral, economic, and environmental needs of outpatients and their families. May include family needs assessment, assessment of safety and environment risk, and assessment of resource needs. - Develops goals related to the social, psychological, behavioral, economic, and environmental needs of the patient and family, and in alignment with the patient’s overall health goals. Identifies potential risks and barriers to positive health outcomes.
