JOB TITLE: Housing Case Manager LOCATION: On-Site: SCHEDULE: Full-Time: JOB POSTING: Provides therapeutic case management services for Coastal Horizons housing, homeless and residential levels of care. Duties include screening, admission, orientation for these programs. As well as assessment of client’s life domain areas; linkage to services in the community; income assistance and employment; assistance coordination of service linkage; medical and medication needs in coordination with client medical needs; home visits/ inspections; gather, enter, and document client information and interactions; and participation in local Continuum of Care meetings in order to enhance communication and the continuity of care. Case manager will also provide or arrange transportation for clients to and from treatment, medical appointments, and other community resource appointments, and monitor client compliance with program expectations.
