What You Will Do: Work collaboratively with the care team using a person-centered care approach. Conduct and maintain accurate resident assessments, care plans, and documentation; communicate updates as needed. Implement and evaluate care plans, adjusting as required. Participate in resident care conferences and support scheduling. Assist with resident transitions including move-ins, discharges, and transfers. Provide direct nursing care and administer medications and treatments in compliance with policies and regulations.
