Department: 34316 Advocate Lutheran General Hospital - Diabetic Education Status: Part time Benefits Eligible: No Hou rs Per Week: 0 Schedule Details/Additional Information: Days primarily M-F but may be some weekend days in the future. Pay Range: $35.50 - $53.25 Major Responsibilities: Responsible for planning, implementing, and evaluating diabetes self- management training across the continuum for inpatients, outpatients, significant others, and community. 3)• Collaborates with patients, families, and/or significant others in establishing mutual learning goals and behavior change goals. 4)• Documents evaluation of outcomes, and revises interventions and follow-up plan for patients, families, and/or significant others. 5)• Participates in clinical evaluation of patient care. 6)• Accesses resources to facilitate discharge planning and appropriate follow-up. 7)• Bases instruction on current standards of practice and input from physicians. 8)• Participates in planning and conducting community service projects, programs, and support groups. 9)• Teaches, evaluates, and provides patient follow-up specific to diabetes classes. 10)• Communicates education and follow-up recommendations to...
