Also responsible for the care of Home Health Services patients and the allocation of agency resources. ESSENTIAL DUTIES AND RESPONSIBILITIES: - Makes initial patient visits, recertification visits, resumption of care visits, Home Health Aide (HHA) and Licensed Practical Nurse (LPN) supervisory routine visits, and discharge visits. - Consults with the attending physician in establishing and updating the Plan of Care. - Participates in the Home Health Services Interdisciplinary Group meetings as appropriate. - Completes OASIS documentation at Start of Care, recertification, resumption of care, transfer, and discha...
