Interim Housing Case Manager

Job description

Position Summary This position is responsible for providing direct clinical and case management services, including: intake and assessment with new tenants, collaborative development of individual service plans with tenants, crisis intervention, individual counseling and providing therapeutic group activities to help tenants meet their goals. The case manager provides linkages and coordinates resources/referrals. Case managers assist tenants in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals. Essential Duties and Responsibilities: Participant Goal-Setting: - Assess participant physical, emotional, and financial needs through intake and on-going individual meetings. - Utilize motivational interviewing techniques to determine participants' readiness to change and plan appropriate interventions - Assist participants with prioritizing self-identified goals and planning to achieve those goals. - Evaluate progress related to goals monthly - Provide necessary resources to participants. - Provide support and guidance to participants through process of goal-achievement.
