Job DetailsJob Location: Community Support Services - Akron, OH 44311Position Type: Full Time (40 Hours)Salary Range: $19.00 - $25.00 HourlyThe Opportunity: We are seeking to hire a Case Manager – ACT Team. This individual provides ongoing community psychiatric support treatment services for a select group of clients who have a serious and/or persistent mental illness co-occurring with substance abuse disorder. This Case Manager will provide training, assistance, and support to help this client receive wrap-around rehabilitation services in the community. Our ACT Teams work together, meeting frequently, to facilitate an integrated, multidisciplinary team approach to service regarding progress, general problems, and concerns pertaining to clients served and their community adjustment. What You’ll Do: Collaborate with the client and the Service Coordinator to develop treatment plans that shall include services and assistance necessary to achieve recovery goals, including an explanation of the availability, intensity, and duration of each service.
