See additional Benefit information below. Scope of Position: The Senior Case Manager (SCM) oversees the overall coordination and direction of services for the people enrolled in services. The SCM monitors the people supported and ensures they remain focused and support them to successfully achieve goals and complete program requirements. The SCM helps assigned people enrolled to obtain additional resources, intervenes in crises; and provides person-centered support and services and facilitates the learning process.
