Job Type Full-time Description: This is a Part Time Position · Under the supervision of the Regional Service Manager, performs duties relating to housing placement Housing Support Specialist will work with clients in need of short-term housing or long-term housing and identify stable living for them Work closely with the Housing Placement Specialist, providing support to PWH towards permanent stable living Tasks/Duties/Responsibilities: 70% Client Services: · Screen clients referred for (Housing Placement Assistance) HPA and identify individuals in need of housing assistance, using agency intake and assessment forms; · Identify permanent housing for clients. · Inspect apartments utilizing Hyacinth standard checklist; · Assist clients with placement in transitional housing. · Negotiate with landlords and provide complete assistance with lease and other documents. · Documentation of baseline viral load, CD4 count, AIDS diagnosis and antiretroviral prescriptions for 100% of clients. Conduct counseling sessions with clients pertaining to housing needs, options, and social service referrals.
