Employment Type: Full time Shift: Day Shift Description: RN HOSPICE-Care Manager for Nursing Homes in the Capital Region Come experience the rewards of working in end-of-life care! NO WEEKENDS, NO ONCALL The Community Hospice has a wonderful opportunity for a FT RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes. Monday - Friday 8a-4:30p: Position Highlights: - Case managers are assigned specific nursing homes to minimize travel - NO WEEKENDS, NO ONCALL - National pre-taxed mileage reimbursement - Meetings and charting conducted via supplied lap top - Comprehensive orientation provided - Full benefits through St Peters Health partners What you will do: - As an RN you will case manage patients in the Nursing home setting and respond to symptom management needs of our Hospice patients and coordinate with our team to provide added support to residents and families. - Work days are Monday to Friday 8 am to 4:30 pm.
