ABOUT THE JOB: The role of the hospice social worker is to deliver varied social work services to Hospice patients and their families. This may include emotional, spiritual, and psychosocial assessments, as well as ongoing counseling, bereavement services and community education, outreach, and referral. The hospice social worker is an integral part of the hospice interdisciplinary team. This is a casual position with no set hours per week. - Functions as a member of the Hospice Interdisciplinary Team - Performs initial psychosocial assessment on every hospice patient to identify needs, determine the medium of service delivery, and complete follow through to ensure provision for social, emotional, and psychological needs of the patient and family. - Provides psychosocial input into the development of the patient plan of care. - Assist the team in understanding the significance of psychosocial factors on the overall health and well-being of the family unit facing serious illness. - Assists with the emergency or direct placements to SNF/NF/AFL facilities as needed. - Provides orientation and ongoing education to staff and volunteers on social work principles. - Completes necessary for...
