Employment Type: Full time Shift: Day Shift Description: REQUIREMENTS: BSN and a minimum of 1 year of experience in behavioral health care OR RN and 2 years of behavioral health care experience required. PA Registered Nurse License: Act 33/34/73 Clearances: Pre-employment drug testing: Position Summary: The Nurse Care Manager provides medication services, triage, and case management to patients at the health center and medical respite program. This role includes supportive therapy, therapeutic interventions, nursing assessments, and other essential nursing services. As an active member of the treatment team, the Nurse Care Manager ensures individualized, patient-centered care and maintains clinical records in eClinicalWorks. Essential Duties & Responsibilities: Build trusting relationships with patients through high-touch engagement. Perform medical triage in collaboration with healthcare staff. Accept and process referrals for the medical respite program. Educate patients and hospital staff about the Medical Respite program and participation expectations.
