Per Diem Licensed Practical Nurse (LPN) Department: Health Center: Employment Type: Flexible: Location: Grand Rapids, MI: Reporting To: Jessica Mays: Compensation: $35.23 - $41.60 / hour Description: Schedule : Per Diem (flex) | Work an average of 3 shifts per calendar month, 2 shifts must be weekend shifts. Schedule based on the needs of the department. Shift Premiums: Weekend (Fri 11:00 pm - Sun 10:59 pm) $ 0.50 Department: Health Center | Porter Hills Village Pay Range: $35.23 - $41.60 (this rate is inclusive of the $3.40/hr. Michigan Skilled Nursing Facility Direct Care Worker Pass-Through approved rate; current approval through September 30, 2026) What You'll Do: As a Licensed Practical Nurse, you’ll provide direct, hands-on care to residents, including administering medications, performing treatments, and monitoring overall health.
