Job description

Title: Program Case Manager Schedule: Monday-Friday 8am-4pm: Job Summary: Provides supervision, opportunities, and skill development to participants served in NeArc’s community-based day and work support program. Provides supervision, guidance, and training to assigned program case managers. Major Responsibilities: Daily Data Monthly/Quarterly Progress Notes/6-month summaries in HCSIS - Evaluate individuals progress periodically and make adjustments as needed to improve outcomes - Evaluate and determine program participants’ interests and develop individual skills. - Provides assigned caseload oversight by attending scheduled meetings, maintaining regular and professional contact with the participant’s team, completing all required documentation, and ensuring the participant’s progress in the program as determined by his or her goals. - Creates individual goals to reflect the participant’s vision regarding skill development. - Maintains all required documentation within required timeframes to include: Assessments: Qualifications: Work Environment: The environment consists of office space, program areas, volunteer sites, vehicles and various locations out in the community.
