Job description

Overview Manage the day-to-day aspects of this program, including managing the program timeline and goals outlined, supervising the Patient Navigators (Peer Counselors) and ensuring accurate record keeping, data tracking, and proper follow-up on all test positives received. Check-in regularly with the Patient Navigators to ensure they are completing their assigned duties, and to monitor the linkage to care activities. Work with the data analyst on dissemination and poster/publication submissions. Responsibilities: - Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employees - Contributes to and participates in the Performance/Quality Improvement activities of the assigned department.
