Job description

The RN Oncology Navigator is accountable for the coordination and continuity of services for patients receiving multi-modality cancer treatments. The RN Oncology Navigator works within a multidisciplinary team to manage and coordinate all aspects of care for a defined population of patients. They assess and plan, in collaboration with the patient, family, and interdisciplinary healthcare team members to meet the cancer care needs of the patient spanning the continuum of care. This may include activities such as coordinating appointments between specialties, regions or sites, acting as a liaison for patients and their families, providing patient education and assessing barriers to patient care. Provides direction to other allied health staff as applicable. Care is provided in a holistic, patient-centered approach with an emphasis on assisting patients to seek an appropriate level of service when needed and reducing the overall cost of care.
