As an Extended Care Nurse at Huron Valley PACE, you’ll provide essential after-hours and weekend care, ensuring participants receive timely, compassionate support when they need it most. Working a unique 7-on/7-off schedule, you’ll take nursing and intake/referral calls, assess patient needs, and coordinate both scheduled and as-needed visits—helping manage complex conditions and prevent unnecessary hospitalizations.
