This position will focus on implementing activities within the Infant Mortality Workplan to reduce chronic diseases and conditions, to include developing and managing interventions that modify behavioral risk factors in diet, physical activity, early prevention screenings, and healthy weight management during pregnancy in an effort to mitigate poor pregnancy and birth outcomes. This includes assessing, planning, implementing, coordinating, monitoring and evaluating services to meet the needs of the identified population.
