POSITION SUMMARY: Work with assigned RN Case Manager in a supportive role to promote efficient and effective management of patient care within the LPN scope of nursing practice. Assist in completion of tasks required to ensure accuracy and timeliness of clinical documentation and coordination of care within the nursing team. Licensing required. Essential Functions Statement(s): - Assist Case Manager with phone calls, comm notes in computer, & prepare information needed for weekend staff. - Daily follow up with hospital for status update of our patients admitted. - Contact field staff with all information that affects patient’s Plan of Care. - Print field staff’s daily/weekly schedule and other pertinent information. - Daily check in with field staff for missed visits adjusting schedule as needed. - Fax MD required information (i.e. missed visit report, fall sheet, abnormal, etc.). - Prepare wound...
