Reporting Directly to the Hospital at Home Director of Nursing, The Hospital at Home Staff RN is responsible for providing high-quality patient care to various patient populations within the framework of the BILH Hospital at Home program. The program provides inpatient-level care for patients in the comfort of their home. The Hospital at Home Staff RN is a Field Based Nurse with some virtual nursing in a centralized command center located in Charlestown. This is a fully on site position. Field assignments can be within a 25 mile radius of any BILH hospital. Job Description: Essential Duties & Responsibilities including but not limited to: - Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home - Navigating patients and families across the continuum of care - Monitoring and responding to biometric data per Hospital at Home Command Center protocols - Conducting audio/visual patient care visits as directed by the care team and/or protocol - Ensuring all interventions in the patient’s care...
