Description Job Summary: Community Health Workers (CHWs) are the backbone of community health at Ellenville Regional Hospital. These vital members bridge the gap between residents and healthcare, ensuring everyone has access to the resources they need to thrive. They act as health education champions, providing culturally sensitive information on disease prevention and healthy living. They also advocate for the community's well-being, identifying and addressing barriers to optimal health. Furthermore, CHWs serve as trusted guides, helping residents navigate the healthcare system and manage chronic conditions. By fulfilling this multifaceted role, Ellenville Regional Hospital's CHW plays a key part in delivering the mission of the Ellenville Regional Rural Health Network (ERRHN). They organize and deliver grant-funded outreach programs outlined in the network's work plans, directly addressing the community's specific health needs.
