The Opportunity: As an Outbound Care Coordinator within the Get Moving Outbound program , you will support Plena Healthcare’s proactive customer care review initiatives. This includes engaging with customers, confirming their interest in services, and coordinating wellness and mobility assessments. Working from our Norwood office, you’ll join a supportive and collaborative team that values initiative, communication, and customer outcomes. Key Responsibilities: - Proactively contact customers to determine their interest in participating in a wellness assessment conducted by Plena Healthcare - Clearly explain services and obtain appropriate customer consent for assessments and follow-up care - Coordinate and schedule mobility and wellness assessments where appropriate - Create and maintain well-planned, optimised schedules for clinicians delivering a...
